POSITION

RN Case Manager


PURPOSE

Serve as a dedicated Home Health Nurse, overseeing comprehensive care coordination for assigned caseload and provide skilled care nursing visits in the Denver metro area. With this role, you have the freedom to choose the pace at which you grow your hours from part-time to full-time, making it an ideal opportunity for those looking to gradually expand their schedule while achieving a full-time position within a flexible timeframe. Continuously assess client needs, coordinate of care with various providers, and ensure top-quality healthcare. Deliver skilled nursing services, provide consumer education, manage documentation, and supervise care providers to guarantee exceptional and personalized care.

Brief Job Description:

  • Complete home visits every 60 days or more frequently, as client condition warrants, to reassess client needs for CNA and Skilled Nurse services
  • Perform skilled nursing services within the RN scope of practice.  
  • Complete orders and resumption of care assessments in client home, within 48 hours of hospital discharge
  • Provide ongoing nursing case management support to client and caregivers related to underlying client diagnosis and symptoms
  • Coordinate with other disciplines to provide quality care to client
  • Provide Annual Competencies to CNA’s on CNA tasks provided to client, as required by state and federal regulations
  • Collaborate with Clinical Managers and other team members in the coordination of services
  • Manage communication with physicians, specialists, and other client care team members to ensure orders are up to date, PASCO employees are appropriately trained, and orders are reflected in client Care Plans.

Qualifications:

  • Minimum 1-year nursing experience required
  • Home Health nursing experience highly preferred, but not necessary
  • Experience with the disability community highly preferred, but not necessary
  • Self-motivated and autonomous
  • Excellent Communication & Organizational Skills


Education and/or Licenses:

  • Active and unencumbered license to practice as an RN in Colorado (will be confirmed in DORA)
  • Current CPR / BLS

        

Job Type: Full-time (32 hours per week is considered full-time in this role)

Pay Range: $32-$42/hour DOE + sign-on bonus ($5,000 for full-time) + mileage reimbursement
